As a Registered Nurse in the Nephrology Clinic, you will provide family-centered care, supporting children with kidney-related conditions. This role involves clinical assessment, telehealth, care coordination, case management, and patient education.

Key Responsibilities:

  • Develop nephrology expertise: Gain knowledge on kidney function, disease management, diagnosis, and treatment.

  • Clinical Assessment: Monitor patients' kidney conditions in-person and via telehealth (phone, email, MyChart).

  • Care Coordination: Manage patient care activities, referrals, test follow-ups, and liaise with interprofessional teams.

  • Patient Education: Educate families on kidney health, treatments, and wellness strategies in a culturally sensitive and developmentally appropriate manner.

  • Virtual Care Support: Use telehealth technologies to triage patients, ensuring timely access to the right level of care through clinical questioning, judgment, and protocols.

Required Skills & Qualifications:

  • Graduate of an accredited Baccalaureate Nursing Program.

  • Registered Nurse (RN) license in good standing with the College of Nurses of Ontario.

  • 2-3 years of nursing experience (pediatrics, ambulatory care, or nephrology preferred).

  • Experience with Epic (electronic health records) is an asset.

  • Strong critical thinking, problem-solving, and teamwork skills.

  • Excellent verbal & written communication skills for effective interaction with patients, families, and healthcare teams.

  • Ability to make independent decisions in a fast-paced ambulatory care environment.

  • Commitment to lifelong learning and staying updated on latest nursing practices and research.
