As a Registered Nurse in the Nephrology Clinic, you will provide family-centered care, supporting children with kidney-related conditions. This role involves clinical assessment, telehealth, care coordination, case management, and patient education.

Develop nephrology expertise: Gain knowledge on kidney function, disease management, diagnosis, and treatment.
Clinical Assessment: Monitor patients' kidney conditions in-person and via telehealth (phone, email, MyChart).
Care Coordination: Manage patient care activities, referrals, test follow-ups, and liaise with interprofessional teams.
Patient Education: Educate families on kidney health, treatments, and wellness strategies in a culturally sensitive and developmentally appropriate manner.
Virtual Care Support: Use telehealth technologies to triage patients, ensuring timely access to the right level of care through clinical questioning, judgment, and protocols.
Graduate of an accredited Baccalaureate Nursing Program.
Registered Nurse (RN) license in good standing with the College of Nurses of Ontario.
2-3 years of nursing experience (pediatrics, ambulatory care, or nephrology preferred).
Experience with Epic (electronic health records) is an asset.
Strong critical thinking, problem-solving, and teamwork skills.
Excellent verbal & written communication skills for effective interaction with patients, families, and healthcare teams.
Ability to make independent decisions in a fast-paced ambulatory care environment.
