This role is pivotal in coordinating admissions, discharges, and transfers to ensure timely and efficient patient care.

Key Responsibilities:

Patient Flow Coordination:

  • Function as the primary facilitator of daily patient flow.
  • Oversee admissions, discharges, and transfers across all hospital units.
  • Collaborate with the Admitting department to manage preferred bed accommodations.
  • Ensure timely transfers between units and departments.

Emergency & Specialty Unit Coordination:

  • Work with ICU, Pediatrics, Mental Health, and Addictions departments to support patient transitions.
  • Conduct regular rounds in the Emergency Department to expedite admissions and bed placements.

Stakeholder Communication & Collaboration:

  • Liaise with physicians, hospitalists, resource nurses, and discharge coordinators to optimize patient movement.
  • Communicate with Home and Community Care Support Services to facilitate timely discharges.
  • Lead daily bed meetings and initiate bed alerts when needed.

Operational Leadership & Problem-Solving:

  • Identify and address bottlenecks in patient flow.
  • Implement strategies to reduce congestion in emergency and admitting areas.
  • Maintain hospital-wide awareness of current patient volume and capacity.
  • Support a safe and efficient work environment for patients and staff.

After-Hours & Emergency Support:

  • Conduct hospital-wide walkabouts to identify and resolve issues.
  • Respond to operational challenges, patient complaints, and staff concerns.
  • Act as a resource for emergency codes and escalate issues when necessary.

Qualifications:

🎓 Education & Experience:

  • Registered Nurse (RN) with College of Nurses of Ontario.
  • BScN or health-related discipline (Master’s preferred).
  • Minimum 5 years of acute care hospital experience.
  • 2 years of leadership experience in a hospital setting.
  • Postgraduate diploma in Utilization Management (asset).
  • Experience in utilization review (preferred).

Additional Information:

📅 Work Schedule:

  • Days/Evenings/Nights, including weekends and statutory holidays (subject to operational requirements).

💰 Annual Salary:

  • Minimum: $104,227.50
  • Maximum: $130,279.50
