Description:

The Consultant will be responsible for leading engagement activities with various stakeholders from across the health system to inform the development and implementation of policy and program changes. The Consultant will need to understand and interpret data to design targeted interventions and drive measurable improvements in performance. The Consultant will need to build and maintain strong relationships with providers and partners to enable and support challenging process and organizational changes aimed at improving system capacity and outcomes. The Consultant will lead evaluation activities for key initiatives, including the identification of key performance indicators and the collection and analysis of data to measure and assess impact.

The healthcare landscape is complex; the Consultant will work as part of the OH team to ensure that OH is a key player in the partnerships required for integration and transformation of health care services.

Here is what you will be doing:

  • Leads the development and achievement of Regional strategies including (but not limited to):

    • COVID response and recovery initiatives, including testing and vaccination programs

    • Strategic partnership projects

    • Mental health and addictions priorities, including opioid overdose, supportive housing, centralized access models, and crisis response

    • Health equity initiatives, including the implementation of an anti-racism framework across the Region’s health system, and an engagement strategy for Indigenous and Francophone communities, as well as patients/family/caregivers.

  • Leads broad engagement and facilitation activities (with senior level health service provider staff, physicians, patients/caregivers) to ensure policies and strategies are co-designed with service users and aligned with partners’ objectives.

  • Analyzes and interprets robust data sets to understand service gaps, risks, and challenges, and identifies and engages appropriate partners to problem solve, design, and implement solutions.

  • Leads evaluation activities for key initiatives, including the identification of key performance indicators and the collection and analysis of data to measure and assess impact.

  • Is the subject matter expert for key elements of the health system (e.g., community support services, mental health and addictions, etc.) in order to provide advice and identify levers or programs that can be optimized to meet OHN Region objectives.

  • Leads internal cross-collaboration (Digital, Analytics, Clinical, Performance/Finance) to drive system strategy informed by data and evidence and enabled by digital and funding opportunities.

  • Works across sectors and outside of healthcare to identify innovative and creative opportunities for transformation and integration; drives system change within broader health and social care.

  • Develops and implements a strategic communication plan that promotes transformation practices and ensures system alignment and integration.

  • Ensures Regional strategic projects align with and achieve strategic priorities set out in the organization’s overall Strategic Plan and are tied to its Mission, Vision and Values.

  • Leads, champions, and directs the development of transformation strategies for the organization.

  • Leads, champions, and directs the development of partnership strategies for the organization.

  • Builds and redesigns relationships with external organizations (Ontario Health Teams, HSPs, broader health system partners and patient, family, and caregiver advisors) to advance OH strategic priorities.

  • Employs political acuity, negotiation, and persuasion to enable and strengthen strategic collaborations that advance each partners’ interests and objectives

  • Is a subject matter expert in effective engagement and co-design principles to ensure voices of the Region’s populations are represented in Regional priorities, policy and program development; is able to tailor messaging to broad range of stakeholders.

  • Leverages existing partnerships to further integration priorities and improve access to programs and services, particularly related to health and housing (shelter, community housing, supportive housing)

  • Central point of contact for key partners (e.g., patient, and family advisors, Indigenous or Francophone providers, etc.) to respond to and resolve urgent issues

  • Ensures that the partnership strategy will align with and achieve the strategic priorities set out in the organization’s overall Strategic Plan and is tied to its Mission, Vision and Values.

  • Collaborates with relevant parties to enhance OH visibility and stature locally, nationally, and internationally, including identifying, taking the lead on and implementing strategic transformation initiatives.

  • Leads discussions and activities to ensure that OH becomes an integral part of the Ontario Health Team model that will work to provide a new way of organizing and delivering coordinated healthcare services in local communities.

  • Represents OH as Lead on multiple Committees.
